Before your initial consultation, we will need you to provide us with some information. For your convenience, we have posted some forms here on our site. Click on the link below to complete the forms on-line. You may then print them out and bring them to our office on your first visit. If you prefer, you also may complete the paperwork in our office. We ask that you bring your insurance information with you at this time.
Your first visit to Le Cara Chiropractic will include a consultation with the doctor to discuss your health concerns and/or reasons for your visit. We will review your health history and do an initial examination to determine if chiropractic care is appropriate for you. Following your exam, Dr. Le Cara will discuss the results and give you a recommendation for treatment.
After your initial consultation, the doctor will give you a comprehensive physical examination that includes standard orthopedic, neurological and chiropractic tests. In some cases, x-rays may be necessary to either rule out more serious conditions or assist us in developing a methodology that will best address your health needs. At this point, Dr. Le Cara will either propose a treatment plan, or he may give you treatment options.
Dr. Le Cara works closely with other health care professionals and, if necessary, he may refer you for additional testing procedures to ensure that your condition is accurately diagnosed.
Dr. Le Cara uses a timeline approach to treatment. Before treatment begins, he will discuss what kinds of improvements you can expect to see and when you can expect them. This provides a gauge by which you can determine if you are achieving your health and wellness goals through a particular course of therapy or treatment.
If no further testing is required, you will be given your first treatment during your first visit. This may include spinal adjustments, physical therapies and/or soft tissue therapies.
Chiropractic care is covered by most health insurance policies. Le Cara Chiropractic participates in many insurance programs and personal injury cases and also accepts some worker’s compensation cases, at the discretion of the doctor. For new patients, payment is due at the time of the service (cash, check, MC/Visa). For established patients with insurance, we will bill your insurance company when furnished with the provider information. Click on the above link to check on your coverage.
